What to do if you've Lost your Passport

Losing anything can be stressful and frustrating, but losing something as important as a US issued ID can be so much worse. This Gizmo Guide is here to help guide you in the right direction in the event of passport damage or disappearance

Prerequisites: Must be a US Citizen and have possessed a passport before

Difficulty Level: Member • Nerd • Platypus

Estimated Time: A few hours (Depends on government facilities wait time)

Expected Outcome: You will be able to work through this process with a better understanding, and without wasting time

Tools Required: Evidence of citizenship and another government-issued, valid photo ID

Steps to Perform 

  1. You must go in person to an acceptance facility or passport agency

  2. Fill out the lost passport form (DS-64)

  3. Fill out a new passport form (DS-11)

  4. Provide citizenship evidence (birth/naturalization certificate)

  5. A photocopy of that citizenship evidence

  6. Your current ID (drivers license or similar)

  7. A photocopy of your current ID

  8. A photo for your passport (some places will take your picture there)

  9. Any associated fees for the process
