Tools Required: Evidence of citizenship and another government-issued, valid photo ID
Steps to Perform

You must go in person to an acceptance facility or passport agency

Fill out the lost passport form (DS-64)

Fill out a new passport form (DS-11)

Provide citizenship evidence (birth/naturalization certificate)

A photocopy of that citizenship evidence

Your current ID (drivers license or similar)

A photocopy of your current ID

A photo for your passport (some places will take your picture there)

Any associated fees for the process
